It is designed to help estimate costs for you and your covered dependents for each of the Aetna medical plans based on specific needs. The following tips are designed to help you navigate the eValuator. If you need assistance with this tool, please contact your local human resources department.LoginEnter your employee ID in the login field. Your password is the last four digits of your Social Security Number and your date of birth -- MMDDYYYY. (Example: 278605221972)After logging in for the first time, you must change your password. Your new password must be at least seven characters and contain at least one number and one letter. The password you select is case sensitive. Please contact your local human resources department if you cannot log on.Choosing Medical ServicesEnter information for your spouses and children ONLY if you plan on covering them as qualified dependents. The terms "basic," "moderate," and "complex" refer to three general levels of health care usage -- not the plans. Select the usage level that best describes you and your eligible dependents' anticipated health care needs. If you want to modify the general usage level so that it is more specific, move your cursor over the "Modify Services" button and click on one of the services. Specific CostsThe eValuator is an estimator, so costs provided for services are based on national health care averages.Prescriptions should be entered as the number of refills. On the Aetna Choice Plan, non-formulary drugs are calculated at 30 percent of $106, the national average cost. Prescriptions exceeding the national average could result in higher costs.eValuator estimates for the following services encompass the complete course of treatment, including physician visits, hospitalization, lab work and other care:* Chiropractic visits * Chemotherapy and Radiation Therapy * Mental Health/Chemical Dependency -- Inpatient * MaternityViewing Your ResultsClick the "View Results" button at any time during your session to view your results. The costs for your services are displayed for all three plans so that you can compare annual employee contributions, out-of-pocket expenses and total annual costs. Before you make changes by modifying services, you may want to write your results on paper or print the screen to keep track of your results.Click on the "View Tax Savings" button to use the eValuator's "Flexible Spending Account" tool to estimate how much you could save in taxes through the health care reimbursement plan. Please note that once you have completed using the eValuator, your information is not saved.
